Well this should be done with the code itself, and then in the FGD file for the mod you are creating.
For example if you open scientist.cpp in the Spawn function you will see a switch for selecting the head. Most probably your monster is based on scientist or human grunt.
I see, ok then try my modified hgrunt as base.
You might have to re-define some entries... If you're using spirit as base, there should be no need of extra work.
